NHS continuing healthcare (CHC) is a package of care arranged and funded solely by the NHS for individuals who are not in hospital, but who have on-going complex, intense, or unpredictable healthcare needs. You can receive CHC services in any setting, including your own home or a care home.

If you are eligible for CHC, Bristol, North Somerset and South Gloucestershire CCG will fund the care required to meet your identified care needs.

Who is eligible for NHS continuing healthcare?

Eligibility is not dependent on a particular disease, diagnosis or condition, or on who provides the care or where it’s provided. If your overall assessment of care shows that you have a ‘primary health need’, you may be eligible.

Anyone over 18 years old who has been assessed as having a certain level of care need is entitled to NHS continuing healthcare. CHC is free, unlike support provided by Local Authorities where a financial charge may be made depending on a person’s income and savings. Assessments for adults are made according to the Department of Health National Framework.

COVID-19 update

Following the government announcement on 5th July in relation to the COVID-19 restrictions, the CCG Funded Care Team has reviewed its arrangements in relation to the way assessments, reviews and resolution meetings are held. In order to continue to offer the highest level of protection to patients, families and staff, the Funded Care Team will continue to use remote technology (telephone, video calls etc.) to deliver the service.

Face to face meetings will continue to be offered in exceptional circumstances only. This is in line with our policy which is that staff continue to work from home.
