The Freedom of Information (FOI) Act gives anyone the right to request access to all types of recorded information held by public authorities, such as NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board.
The purpose of the Act is to promote greater openness and to help inform people of how public authorities make their operational decisions, and how public money is used.
All requests for information must be made in writing, clearly stating your name and address and what information you would like. Fill out our online contact form or send your request to the FOI Manager at the above address. Or you can email bnssg.foi@nhs.net
We will acknowledge your request within three working days and you can expect to receive a response within 20 working days.
Subject access requests
The Data Protection Act gives you the right to obtain a copy of information that NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board holds about you and the reason why.
If you would like to receive a copy of information we hold about you your request should be made in writing by post to the above address or email: bnssg.foi@nhs.net
When making your request, please include the following details:
- your name, address and postcode
- any relevant case reference numbers
- the type of information or documents you want to look at including any relevant dates, and any preferences you have for the way you would like us to send the information to you (for example, hard copy, large print or by email)
- one form of ID: Passport, Driving Licence or Birth Certificate (if you are requesting records on behalf of someone else separate ID will be required and we will confirm this with you).
