This form must be completed when applying for medications or drugs that are not normally commissioned by the CCG. Please refer to the Interventions Not Normally Funded Lists (INNF) before making an application.
Application form for funding for Nasal Treatment
This form must be completed by the Fertility Service to refer patients who meet criteria to receive Licensed Fertility Treatment. Please send completed forms to the IFR Team.
This form should be completed by GPs and other clinicians who are referring patients to General Infertility Clinics for an initial assessment.
This form must be completed when applying for treatments that are not routinely funded by the ICB.
