In January 2019 we hosted a seminar alongside Bristol Health Partners on Public health and community-based support initiatives: the work of Health Integration Teams.
Bristol Health Partners Health Integration Teams address pressing health challenges to improve health outcomes across Bristol, North Somerset and South Gloucestershire. Health Integration Teams bring together health and care professionals, patients, members of the public, commissioners, local government, third sector organisations and researchers.
