One Weston Care Home Hub nominated for national award
A local care home initiative, the first of its kind in the UK, has been nominated for a national Health Service Journal (HSJ) Award for its support for care home residents in Weston-super-Mare.
The One Weston Care Home Hub was created by the Bristol, North Somerset and South Gloucestershire Integrated Care Board (BNSSG ICB), Weston Locality Partnership and the Pier Health Primary Care Network (PCN) and has been shortlisted for the HSJ’s Place-based Partnership & Integrated Working Care Award.
The service supports care homes in the area to tackle the inequalities experienced by older people, people with learning disabilities and mental health issues in Weston, through collaborative working and supporting care homes with reactive and proactive care delivery.
Since September 2021, early successes of the One Weston Care Home Hub have shown a 45% reduction in emergency hospital admissions, 20% reduction in Emergency Department attendances and an 11% reduction in ambulance callouts to care homes.
It has also boosted job satisfaction among care home staff and has hosted new training posts and leadership development for staff.

The HSJ Awards celebrates healthcare service excellence in the UK, highlighting best practice, improving patient outcomes and promotes innovation that drives improvements to better care.
Winners of the HSJ Awards will be announced at a ceremony in London on Thursday 16 November.
