Children’s community health services

Children’s community health services provide high quality and integrated care services to young people for chronic illness, behaviour, mental health and developmental difficulties.

Bristol and South Gloucestershire

For Bristol and South Gloucestershire residents, Community Children’s Health Partnership (CCHP) provides community children’s health services and children and young people’s mental health services.

This partnership is led by Sirona Care & Health, working with Bristol Community Health, Avon and Wiltshire Mental Health Partnership NHS Trust, University Hospital Bristol NHS Foundation Trust, Barnardo’s and Off the Record.

The services that fall within children’s community services include:

• health visiting
• school nursing
• child and adolescent mental health
• speech and language therapy
• occupational therapy and physiotherapy
  community paediatricians
• and a range of dedicated services for vulnerable children, including children in care, children with learning disabilities, children with life-limiting conditions and children with drug and alcohol problems.

The CCHP staff team provide a high quality and integrated service to children, young people, families and carers, especially those from vulnerable and hard to reach groups.

Council Local Offers

Local Offers created by councils set out in one place information about provision we expect to be available across education, health and social care for children and young people who have special education needs or are disabled. This includes those who do not have education, health and care plans.

South Gloucestershire SEND strategy

We have been listening to the views of children, families, settings, schools, colleges and other stakeholders through engagement events in the autumn term 2017. As a result of this work, a draft Strategy was co produced and consulted on in February 2018.

This Strategy sets outs the vision and strategic priorities for South Gloucestershire local area for the period 2018–2023.

North Somerset

In North Somerset, Weston Area Health NHS Trust (WAHT) provide community health services for children. The service works with young people aged 0 – 19 years and their parents and carers to provide a range of services at specialist level.

The services comprise of:

• community paediatric services
• speech & language therapy
• occupational therapy and physiotherapy
• specialist child & adolescent mental health services (CAMHS).

The teams work in partnership with children, young people, their families and carers and a range of other agencies in North Somerset.
