Adult community health services

Community health services support people to stay independent and well at home or in their own community and avoid unnecessary hospital stays.

Community health services cover an extensive and diverse range of activities and are delivered in a number of different settings, including in people’s own homes, community clinics and community centres. The community health services available to adults in Bristol, North Somerset and South Gloucestershire include:

  • community nursing
  • occupational therapy
  • physiotherapy
  • dietetics
  • end of life care.

On 1 April 2020 Sirona care & health became the sole provider of these NHS services across Bristol, North Somerset and South Gloucestershire (BNSSG) for the next ten years.

Accessing Sirona’s services is simple – just call 0300 125 6789. This Single Point of Access (SPA) is a non-emergency number available 24/7 from 1 April 2020. You should use if:

  • you are receiving adult community health services in your home in Bristol, North Somerset and South Gloucestershire.
  • you are a clinician wanting to refer someone to Sirona’s services.
  • you need support to access the right services to meet your needs.
