Better care, closer to home: adult community health services

From 2020, we aim to make sure that community health services for adults throughout Bristol, North Somerset and South Gloucestershire are more consistent and joined up.

Why is this important?

Community health services support thousands of people every year in Bristol, North Somerset and South Gloucestershire to stay well, independent and avoid unnecessary hospital stays.

In September 2019 we confirmed Sirona care & health as the single provider of community health services for adults across our local area. This will mean:

  • more consistent care for patients and carers and better joined-up working between professionals
  • better partnership working between organisations
  • and improved access to care.

The NHS Long Term Plan, published in January 2019, describes ‘fully-integrated out-of-hospital care’ as one of the biggest NHS priorities for the next decade. It also highlights the value of expanded teams of health and care professionals working together to provide community services that join up around you – rather than making you fit around the way a service works. That’s exactly what we want to achieve with community services in Bristol, North Somerset and South Gloucestershire.
